Monica Sniady was last seen getting on a bus on Therry Road, Campbelltown, just after 9am last Saturday.
When she could not be located or contacted, Monica, who is originally from Victoria, was reported missing to officers attached to Campbelltown City Police Area Command, who commenced inquiries into her whereabouts.
Police have serious concerns for the 56 year old because Monica lives with several medical conditions that require medication.
Monica is described as being of Caucasian appearance, approximately 160cm to 170cm tall, of medium build, with brown hair.
She was last seen wearing a light green hooded jumper with New York written on the front, a black shirt, light pink tracksuit pants or leggings, and a pair of black boots.
At the time she was carrying a clear or white plastic bag of clothing and a green Woolworths fabric shopping bag.
Monica is known to utilise public transport and may be travelling towards Victoria.
If anyone sights Monica or has any information on her whereabouts, they are urged to contact Campbelltown Police or Crime Stoppers on 1800 333 000.

Police are also appealing for information that will help them find a missing teenager from South West Sydney.
Antonio Zuccolotto, aged 16, spoke to family about 8.50pm yesterday (Sunday).
Police say Antonio may have been in the Cabramatta area at this time.
When he was unable to be contacted later, officers attached to Fairfield City Police Area Command were notified and have commenced inquiries into his whereabouts.
Police and family hold concerns for Antonio’s welfare due to his age.
Antonio is described as being of Caucasian appearance, about 182cm tall with heavy build and brown hair.
Antonio is known to frequent the Fairfield West, Cabramatta, Mt Druitt and Blacktown areas.
